While you may not enjoy filling out paperwork, you're gonna LOVE the events and fun activities these forms let you do! Just print them out and return them completed to Jimmy or Jeanette at the church! 

Permission, Medical Consent, and Behavioral Agreement Jan 2011--December 2011
            Required for all activities from Jan 2011-Dec 2011. This form requires a notary. You only need to give us this 
            once a year, and if you were at the January parent meeting and filled it out then, you're good to go!

            Please bring these forms with you to camp! They are you or your camper's ticket to camp!

Urban Rocks Waiver (for climbers under 18)
            The junior high will be hitting up Urban Rocks this summer for some awesome 
            rock/wall climbing...you must have this form completed to climb!
